This is a healing and supportive space for you. The Terms & Conditions below simply outline how we work together, so everything is clear, and you can relax and focus on your wellbeing. If you have any questions, we’re always here to help.
1. Acceptance of Terms
By signing the registration form, the Client acknowledges they have read, understood, and accepted these Terms & Conditions and consent to receive Services from Sunshine Primary Health Pty Ltd.
2. Services & Consent
Services include physiotherapy, massage therapy, podiatry, holistic care (such as Reiki, energy work, or related practices), and other allied health services as provided by Sunshine Primary Health Pty Ltd. Our practitioners aim to provide the highest quality care, but as with all healthcare, outcomes may vary between individuals and cannot be guaranteed. The Client understands the physical nature of treatment (where applicable) and accepts the inherent risks, including but not limited to soreness, adverse reactions, injury, or other complications. The Client may withdraw consent at any time.
3. Privacy
The Client’s personal information is collected and used in accordance with the Australian Privacy Act 1988. Information is held in confidence and used solely to provide care. The full Privacy Policy is available at www.sphclinic.com.au.
4. Photography & Video
By signing this form, the Client consents to the use of photography or video for promotional purposes by Sunshine Primary Health Pty Ltd, provided verbal agreement is also obtained at the time of recording. The Client may withdraw consent at any time.
5. Fees & Payment
Full payment is due at the time of service. If a third party (e.g. Medicare, NDIS, insurance) declines or reduces payment, the Client remains responsible for all fees. Payment methods accepted: EFTPOS, credit card (fees may apply), cash.
6. Cancellations
At least 24 hours’ notice is required to cancel or reschedule appointments. Failure to do so will result in the full fee being charged. Missed appointments will also incur the full fee.
7. Indemnity & Limitation of Liability
The Client agrees to indemnify Sunshine Primary Health Pty Ltd, its directors, employees, agents, and contractors, from any claim, loss, liability, cost, or damage arising from the Client’s failure to provide accurate information, failure to follow advice, or misuse of Services. Liability is limited to injury or loss caused by proven negligence. Sunshine Primary Health Pty Ltd is not liable for any indirect or consequential loss or harm.
8. Client Responsibilities
The Client agrees to provide complete and accurate health information and to inform practitioners of any changes. The Client accepts responsibility for their own decisions regarding care and treatment.
9. Termination of Services
Sunshine Primary Health Pty Ltd reserves the right to decline or discontinue Services at its discretion, if it is in the best interest of the Client or clinic.
10. Changes to Terms
Terms & Conditions may be updated at any time. The most current version is available at www.sphclinic.com.au.
11. Jurisdiction
These Terms & Conditions are governed by the laws of Victoria, Australia.
